Job Title: Physical Therapist - Home Health Specialty: PT Home Health Shift: Day, 5x8-Hour (08:00 - 16:30) Start Date: ASAP Job Duration: 13 to 26-week renewable contract -(monthly income guaranteed) - Description: We are seeking a skilled Physical Therapist for a field position in the Clanton area. As a member of our team, you will provide exceptional care to patients in their homes, helping them recover from injuries and illnesses. Qualifications:

• State's License
• Required: Home Care Home Base, Oasis
• Must have Previous Adult Home Health Experience
• will consider temp to hire
• Mileage Reimbursement IRS Standard
• -Will consider temp to hire
• Mileage Reimbursement IRS Standard
• Must meet 30 productivity points
• OT preapproved by the branch
• No on-call unless stated upfront
• COVID-19 Vaccine (Facility Guideline): -Required - Medical/Religious Exemptions Only
• 2 years of experience in specialty
• will accept locals Responsibilities:
  • Perform comprehensive assessments and develop individualized treatment plans for each patient
  • Provide hands-on therapy to patients in their homes
  • Communicate regularly with physicians, nurses, and other healthcare professionals to coordinate care
  • Document patient progress and outcomes in an accurate and timely manner
  • Educate patients and their families on home exercise programs and other self-care strategies
