designing, executing, and assessing individualized patient care

based on client needs): Conduct evaluations and assessments to
determine patient eligibility for care. Leverage available tools
and resources to create tailored care plans that specify the
appropriate level and frequency of treatment. Engage with the care
team, physicians, payors, patients, and families to address care
requirements and implement the patient’s care plan. Provide
high-quality skilled care while overseeing the activities of other
team members. Maintain comprehensive daily documentation of care
provided in our electronic medical record (EMR) system. Foster a
culture of compassion through personal accountability and teamwork.
