Registered Nurse (RN) | Home Health Location: Tuscaloosa, AL Agency: Cynet Health Pay: $2,226 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/16/2026 About the Position TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Tuscaloosa, Alabama, 35401! Job Title: Home Health Visit Nurse Profession: RN Specialty: Home Health Duration: 13 weeks Shift: 08:00 - 17:00 Hours per Shift: 9 Experience: Minimum of 1 year in home health License: Registered Nurse (State of Alabama or Compact license) Certifications: AHA BLS or Red Cross Must-Have: - Associates Degree in Nursing - 1 year of experience in home health nursing - Experience with Homecare Homebase (HCHB) Description: The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines. Provides skilled care, education, treatment, and emotional support to patients. Monitors and assesses the care provided while ensuring the patient attains and maintains their highest level of functioning. Ensures that the physician and all members of the care team are aware of pertinent changes in the patient's condition and progress toward goals. Collaborates with the Home Health RN Care Manager to ensure that the established plan of care is appropriate and is implemented or modified based on the patient's responses to treatment. Provides skilled nursing care to an assigned caseload of patients. Monitors the care, treatment, and service provided by other disciplines to ensure the patient receives the necessary care to improve their health status. Educates patients and their families on proper home health care strategies and procedures. Instructs and supervises the patient and family in the care of the patient and maintenance of a healthy environment. Evaluates and revises the plan of care in collaboration with the Home Health RN Care Manager based on patients' identified needs and progress towards goals. Reviews the plan of care to ensure that physician orders are followed. Performs skilled assessments and documents findings accurately and timely. Maintains an accurate and current list of each patient’s medications. Communicates regularly with the Home Health RN Care Manager regarding patients’ progress towards goals, barriers to care, and outcomes. Establishes positive working relationships with physicians and keeps them informed of patients' issues and progress. Assesses patient outcomes and suggests ways to improve outcome scores. Actively participates in performance improvement processes to ensure positive patient outcomes. Participates in interdisciplinary team meetings and on-call rotation as needed. Physical Factors: Able to lift 50 pounds and position patients weighing over 200 pounds. Ability to stand, walk, kneel, sit, crouch, or stoop with good balance for extended periods. Must be able to maneuver over uneven surfaces including fields, gravel, and concrete. Good manual dexterity and communication skills are essential. Must be licensed to operate a motor vehicle and tolerate a variety of environments. Physical presence onsite is essential. Hearing and vision must be normal or corrected to within a normal range.
