Job Description:

of inpatient rehabilitation hospitals, home health agencies, and
hospice agencies. As a Case Manager II, you will be instrumental in
developing, implementing, and evaluating personalized patient care
plans. This role is pivotal in facilitating effective communication
between patients, family members, and the healthcare team to ensure
seamless transitions and exceptional care outcomes. Assess new and
returning patients to determine appropriate level of care and
necessary services. Create, implement, and revise comprehensive
care plans based on patient-specific needs and medical history.
Coordinate with healthcare professionals to streamline care
transitions, schedule follow-up appointments, and manage patient
health outcomes. Educate patients and their families on the care
process, post-treatment options, and health maintenance strategies.
Actively participate in case reviews and consultations with other
healthcare team members. Maintain detailed records of patient
interactions, treatments plans, and outcomes for continuous care
and compliance with regulations. Strong interpersonal and
communication skills Proficiency in healthcare management systems
Ability to work collaboratively in a multidisciplinary team Strong
organizational skills and attention to detail Crisis intervention

Certified Case Manager (CCM) preferred Valid nursing or clinical
social worker license
