Job Description:

at home. We are currently seeking an experienced Case Manager II to
manage patient care coordination in our inpatient rehabilitation
hospitals. The ideal candidate will bring a compassionate approach
to patient care and a rigorous attention to detail, ensuring a
seamless and effective transition through different levels of care.
This role demands a skilled communicator with a profound
understanding of healthcare management processes. Assess patient
care requirements and develop individualized care plans in
conjunction with the medical team. Coordinate with healthcare
professionals and departments to streamline care transitions and
delivery. Monitor patient progress and update care plans
accordingly. Communicate effectively with patients and their
families regarding treatment plans and progress. Ensure compliance
with healthcare regulations and standards in patient care
management. Document and maintain accurate patient records and
reports for medical staff and insurance purposes. Strong
organizational and communication skills In-depth knowledge of
healthcare regulations and policies Ability to work independently
and in a team Proficient in case management software and record
keeping Strong empathy and interpersonal skills Medical Insurance
Dental Insurance Vision Insurance 401k Certified Case Manager (CCM)
preferred Valid nursing or social work license in the state of
practice
