The ideal candidate will bring a mix of compassion, healthcare knowledge, and organizational skills to manage patient cases effectively from admission to discharge. Develop, implement, and review personalized care plans in collaboration with healthcare providers. Coordinate resources and services for patient care and follow up, ensuring alignment with health regulations and organizational standards. Act as a point of contact for patients and their families, providing guidance and advocacy through complex healthcare processes. Collaborate with multidisciplinary teams to assess patient progress and adapt care plans as needed. Ensure complete documentation of patient care and adjustments in line with compliance and best practice models. Provide education and support to patients and their families about health condition, treatment plans, and required follow-up.
