Job Description:
Job Description The Clinical Care Assistant provides direct
patient care in the home setting for individuals recently
discharged from skilled nursing facilities. This role supports
continuity of care by delivering wound care, care management
support, and other approved medical services under the direction of
a licensed Medical Director. The Clinical Care Assistant works
independently in patient homes and plays a vital role in ensuring
patient safety, adherence to care plans, and timely communication
with the broader care team. This position is ideal for a Clinical
Care Assistant who is confident in practicing independently,
comfortable traveling within a defined territory, and passionate
about delivering high-quality, patient-centered care in the home
environment. Provide in-home clinical services to patients
following discharge from skilled nursing facilities Perform wound
care, dressing changes, and monitor healing in accordance with
established care plans and protocols Conduct patient assessments
including vital signs, symptom monitoring, and medication
reconciliation Support ongoing care management by identifying
changes in patient condition and escalating concerns appropriately
Educate patients and caregivers on wound care, infection
prevention, medication adherence, and follow-up care Accurately
document all patient encounters, observations, and services
provided in the medical record Coordinate with providers, nurses,
and care coordination teams to ensure continuity and quality of
care Maintain compliance with infection control, safety standards,
and organizational policies Travel independently to patient homes
within assigned service area Requirements Current Medical Assistant
certification required (CMA, RMA, or equivalent) Minimum 1–2 years
of clinical experience as a Clinical Care Assistant required
Demonstrated experience with in-home and hands-on patient care
Comfortable working independently and entering patient homes alone
Strong clinical judgment, attention to detail, and ability to
follow care protocols Excellent communication and patient education
skills Basic computer skills and comfort with electronic
documentation Travel & Work Environment Full-time, field-based
position Must be willing and able to travel up to 1.5 hours driving
distance from home within assigned territory Reliable
transportation and valid driver’s license required Key Attributes
